Please complete the NHS form and the appropriate questionnaire form. Under NHS regulations the practice now needs to clarify the identity and residence of any new patients registering.
Before we can register a new patient we need to see a driving licence or passport (if born abroad) and a recent utility bill. If you were resident overseas but are now residing in this country we need to see supporting documentation from an employer etc.
